Parties
This Memorandum of Understanding is between Organization A and Organization B.

Purpose
Collaborate on a shared community initiative and coordinate outreach, referrals, and outcome reporting.

Shared outcomes
Increase qualified participation, complete warm referrals, and document shared community outcomes.

Responsibilities of Organization A
Program leadership, staff coordination, and reporting.

Responsibilities of Organization B
Community relationships, participant referrals, and local expertise.

Coordination and review
The parties will review progress quarterly, document decisions and commitments, and identify an owner and due date for follow-up.

Information stewardship
Each party will protect confidential information and share only the minimum data needed for agreed activities and reporting.

Term and changes
This understanding will remain in effect for 12 months. The parties may update it by mutual written agreement or end it with reasonable written notice.
